Chyluria or chylous urine is a condition with the presence of chyle in the urine, which results in milky urine. It's due to communication between the lymphatic and urinary system. Chyle is a body fluid within the lymphatic vessels, which conveys the digested fat from the small intestine to the blood. Chyle consists of the lymph and lipid particles called chylomicrons, which are composed of fats and proteins.
Chyluria can be a debilitating condition that can have an intermittent (waxing and waning) and recurrent course. The main symptom is thick, milky, and possibly foamy urine. Cause of foam in the urine is the protein leak which makes an emulsified solution
Other symptoms and signs can include frequent and burning urination (dysuria), passing fat or blood clots, pinky or red urine due to the blood (hematuria), kidney pain, weakness, weight loss, fever and swelling of the legs or scrotum (hydrocele). Chyluria, by itself, is not life-threatening.
Chyluria is rare in the United States and Europe. It occurs in South Asia, Africa, and Central and South America, mainly in rural areas with the abundance of mosquitoes that spread the parasite Wuchereria bancrofti, which causes filariasis.
In India Himalayan belt, especially the part of Bihar, Uttar Pradesh, Orissa has the maximum number of cases of Filaria and Chyluria. Sporadic cases are also found.
The non-parasitical cause may be tumors, abscesses, trauma, tuberculosis & congenital condition.
Most patients misdiagnosed as spermatorrhoea ( DHATU AANA), and most patients are afraid of dying from kidney disease after seeing white urine or clots. Proteinuria commonly seen in chyluria cases, and weight loss adds anxiety to these patients. Many cases failed to respond to conservative treatment with Diethylcarbamazine (Hetrazan), and equal failure and recurrence noted with surgical procedure Betadine or Silver Nitrate installation in the kidney to obliterate lymphatic fistula.

Here is a case of Bank employee from Begusarai, Bihar, who suffered from Chyluria for more than three months.
He also told us that 15-20 years back, he suffered from Filaria, and that time took Hetrazan and becomes fine.
This time he was at market place got wet and after one-week color changes in the urine. (probably the favorable condition for filarial nematodes to flare up
After waking up in the morning and the evening, 3-4 pm chyle appeared in the urine.
He then visited a urologist, and Dr suggested to take Rasgulla and go for the test, chyle appeared. They advised for Betadine installation at the kidney, and out of fear, he was searching for an alternative solution.
