Taslim from Surat came to consult Dr. Ravi on 22nd Feb 2017. She was having habit of plucking hair and she plucks atleast 400 strands daily. This habit was troubling her and her scalp was visible. Dr. Ravi diagnosed her disease as Trichotillomania (TTM) also known as hair pulling disorder, it is an impulse control disorder characterised by a long term urge that results in the pulling out of one's hair. This occurs to such a degree that hair loss can be seen. Efforts to stop pulling hair typically fail. Hair removal may occur anywhere; however, the head and around the eyes area are most common. The hair pulling urge is so intense that it results in anxiety.
On asking how this started, she told that it started first time in her tuition class. She was sitting and suddenly there was irritation in her scalp. She pulled one hair and she got relief and after that it all started. There was no tension in her life and her menses was also normal. She was good at study in school and college both. Upto 22 years of age, there was no tension in her life. She also developed habit of plucking hair and eating white liquid after plucking hair. In the starting, her family members gave her attention and tried to support her but now no one gives her attention in her family, even her husband. She has 4 kids, all are busy in their work and studies and due to this she feels lonely sometimes. She also complained of Migraine, probably 5 times a week. Once there was so much pain that she had to take injection for it. She prefers injection when there is an unbearable headache as it reduces instantly. When she is empty stomach, the chances of migraine increases. The pain is unbearable and it’s so horrible that she can’t tolerate it. The pain increases with sunlight. On her current condition and it’s circumstances her mind is full with negative thinking. On studying her case in totality, Dr. Ravi started treatment process.
After 21 Days
She was followed up on call on 15th March 2017, she reported that she is out of the city and she is feeling 10% improvement in her condition and she can also see new grown hairs on her scalp which is a positive sign of recovery. She also told that when she is out of the city she doesn’t plucks her hairs.
After 4 Months and 12 Days
When she came for follow up on 4th July 2017, there was an improvement in her condition. 2 days ago, on call she complained of migraine. Dr. Ravi does necessary changes in the treatment process and she got relief. Now she can see new hairs are covering her scalp and she is very happy. Initially when she came for treatment her scalp was totally visible.
After 1 Year , 1 Month and 17 Days
Dr. Ravi followed up on call on 10th April 2018, she reported that she is totally fine since 31st January. She reported that sometimes there is headache but she gets relief soon and the frequency of headache is also very low as compared to past migraine episodes. Previously she used to suffer from migraine for continuously 2 to 3 days. Now she is happy with the best results and she is totally recovered from her habit of plucking hairs. Dr. Ravi suggested her to continue treatment process for total recovery.
After 1 Year , 6 Months and 29 Days
on 19th Sep 2018, Dr. Ravi called Tslim over Whatsapp video call. She was roaming in a market with family and showed her head full of hair.
