Cerebrospinal fluid (CSF) rhinorrhea is a condition where the protective fluid that surrounds the brain finds its way into the nose and sinuses, often appearing as a very watery runny nose.
Most cases of CSF rhinorrhea occur after major accidents where the bones of the face and skull experience significant trauma.
Some patients can develop spontaneous CSF rhinorrhea.
The causes of spontaneous CSF rhinorrhea is unknown, but the condition is associated with obesity and high blood pressure. Any factor which increases CSF pressure may cause CSF leak. It may be an associated feature of Hydrocephalus.
RISK OF CSF RHINORRHEA includes the possibility of developing MENINGITIS an infection of the lining of the brain. Depending on the size of the leak, patients may note a continual salty taste and a sense of drainage in the throat or watery drainage from the nose. Bending over and increased activity can make these symptoms worse.
Many people will also notice changes in vision, headaches, ringing in the ears and dizziness.

DIAGNOSING CSF RHINORRHEA
The diagnosis of CSF rhinorrhea usually requires testing the drainage from the nose for a protein called Beta-2 or Tau transferrin. This protein is only found in brain fluid.
Once there is a diagnosis, there several other tests to determine the location of the leak.
A CT scan ( CT CISTERNOGRAPHY) of the head and sinuses often reveal the source of the leak and helps the surgeon plan any needed surgery.
Medications ( includes diuretics like DIMOX can be used to help decrease the production of the fluid that surrounds the brain (cerebrospinal fluid or CSF). This reduces the pressure and allows the leak to seal.

SURGERY : One may require surgery, which can be performed using small tools and cameras (endoscopes). The surgeon places these tools up the nostrils and into the nose. The surgeon may use skin or bone from the inside of the nose or small pieces of fat from the patient’s abdomen to help repair the leak.

1- A Case from Lucknow
A Case of CSF Rhinorrhea from Lucknow with bony defect in cribriform plate, not only got cured symptomatically but also radiologically.
2-A CASE FROM AGRA
A patient from Agra experienced a sudden, transparent fluid discharge from her right nostril, which left her anxious and concerned. Fearing for her health, she consulted a neurologist, who advised her to undergo an MRI scan. The MRI results showed a defect in her cribriform plate on the right side, allowing CSF to leak into her nasal cavity. Specifically, the MRI revealed a 2.1 mm bony defect through which fluid appeared as a linear streak, noted as hyperintense on T2-weighted images. This report confirmed the presence of CSF rhinorrhea, a rare and often serious condition.
3- A case from Poorniya Bihar
The patient initially mistook his condition for a common cold. However, when the watery nasal discharge persisted, he consulted AIIMS Patna, where CT cisternography revealed:

A female patient from Shahjahanpur presented at my Lucknow clinic, complaining of long-term CSF rhinorrhea. She had already been evaluated by an ENT specialist who clinically diagnosed the leak. However, when a MR cisternography was performed, it failed to show any defect at the cribriform plate or skull base.
